Companies wishing to be considered for a supply agreement are invited to complete Creation Foods Supplier Approval Questionnaire and send back to Qa – Creation Foods Company.

Documents Required:

  1. Certificate of analysis with each lot of the supply
  2. HALAL/KOSHER certificate
  3. Copy of HACCP summary
  4. Specifications
  5. Copies of independent certifications
  6. Statements / certification of conformance
  7. Plant registration
  8. Letter of Guarantee
